Are you or family of member eligible for home care services?
1. Your doctor must order the Home Health Care you or your family member is to receive.
2. The patient must be homebound or normally unable to leave his/her home unassisted.
3. The patient must need one of the following:
a. Intermitted Skilled Nursing
b. Physical Therapy
c. Speech Therapy
d. Continue to need Occupational Therapy
